Collaborative Holistic Care & Support Services provides personalised support across Adelaide and surrounding suburbs for people with disability, as well as eligible DVA clients.
Our approach is informed by more than 20 years of nursing and care experience across mental health, drug and alcohol services, general nursing and aged care. This background gives us a broad understanding of the factors that can influence a person’s daily life, including their health, routines, relationships, environment and sense of independence.
We take the time to understand the whole person rather than focusing only on individual tasks. From everyday assistance to nursing care, skill development and community participation, each support arrangement is developed around the person’s needs, choices and goals.
Life rarely stays the same, and support should not remain fixed when a person’s needs, health or priorities begin to change.
At Collaborative Holistic Care & Support Services, we work collaboratively with each person, their family and wider support network to develop an approach that reflects what matters to them now while remaining flexible enough to evolve over time.
We listen to how you prefer to communicate, what helps you feel comfortable, which routines are important to you and where you would like greater confidence or independence. Your support can then be reviewed and adjusted as your circumstances develop, without losing sight of your choices or personal direction.
