Who we are at Collaborative Holistic Care & Support

Care informed by experience

Collaborative Holistic Care & Support Services provides personalised support across Adelaide and surrounding suburbs for people with disability, as well as eligible DVA clients.

Our approach is informed by more than 20 years of nursing and care experience across mental health, drug and alcohol services, general nursing and aged care. This background gives us a broad understanding of the factors that can influence a person’s daily life, including their health, routines, relationships, environment and sense of independence.

We take the time to understand the whole person rather than focusing only on individual tasks. From everyday assistance to nursing care, skill development and community participation, each support arrangement is developed around the person’s needs, choices and goals.

Our approach

Support that makes room for change

Life rarely stays the same, and support should not remain fixed when a person’s needs, health or priorities begin to change.

At Collaborative Holistic Care & Support Services, we work collaboratively with each person, their family and wider support network to develop an approach that reflects what matters to them now while remaining flexible enough to evolve over time.

We listen to how you prefer to communicate, what helps you feel comfortable, which routines are important to you and where you would like greater confidence or independence. Your support can then be reviewed and adjusted as your circumstances develop, without losing sight of your choices or personal direction.

What guides us

Values you can see in practice

Our values influence how we communicate, make decisions and work alongside every person. They are reflected in the way we listen, respect boundaries, respond to feedback and create opportunities for people to take a more active role in their own lives.

Choice and Control

You have the right to make decisions about your life, your support and the people involved. We provide clear information, listen to your preferences and respect the direction you choose.

Empowerment

We focus on building confidence, capability and practical skills rather than taking over. Support is delivered in a way that encourages participation and recognises what each person can do.

Hand and heart icon

Dignity & Respect

Every person deserves to feel heard, valued and comfortable in their own support arrangement. We respect your identity, privacy, routines, boundaries and lived experience in every interaction.

Inclusion

We believe people should have genuine opportunities to participate, connect and feel part of their community. Support is planned around the activities, relationships and experiences that are meaningful to you.
