A good referral is about more than finding an available provider. It is about finding a team with the experience to understand the person’s needs, communicate clearly and adapt support when circumstances change.
Collaborative Holistic Care & Support Services brings more than 20 years of nursing and care experience across mental health, drug and alcohol services, general nursing and aged care. This gives us a strong understanding of both everyday support and more complex health considerations.
We take referrals seriously from the beginning. Each enquiry is reviewed with care, with attention given to the person’s goals, routines, communication preferences, health needs and current support network. We are also honest about whether our team has the right skills and availability, because the right fit matters for everyone involved.
Referrers need to know that important details will not be overlooked. We take the time to understand the full picture, ask the right questions and keep communication clear throughout the referral process.
Our support can evolve as the participant’s needs change, whether that involves daily assistance, nursing care, skill development, community participation or coordination across a wider support team. With the participant’s consent, we can also work alongside family members, Support Coordinators, allied health professionals and existing providers to help maintain a consistent approach.
Most importantly, the participant remains involved in decisions about their support. Their preferences, boundaries and goals guide how the arrangement is developed.
